Theater Database Application

To gain access to the theater database, groups (including schools) must fill out the following application. Only schools in Canyons School District and established community theater groups within Canyons School District boundaries may apply for access. Each school department needs to apply separately. (e.g.: Dance, Drama, etc.) This application will be submitted for consideration. You will be notified within 10 working days. Access to the database is on a yearly basis from July 1 - June 30 of each calendar year. Applications need to be resubmitted each year. Only individuals listed as a contact person on the application will be allowed access to the database and warehouse. Each organization (or school department) is limited to 3 authorized individuals. Individuals MUST be at least 18 years old and NOT a K-12 student. Access is granted dependent upon adherence to the agreements within.
